The exact mechanism is not fully understood, but the evidence is robust enough that many rheumatologists recommend this dietary shift.

Stay well-hydrated. Adequate water intake supports renal uric acid excretion. Patients on furosemide should work with their physician to determine an appropriate daily fluid intake, as unrestricted hydration may not be appropriate for those with severe heart failure.

Limit fructose-sweetened beverages. High-fructose corn syrup, found in many sodas and processed juices, accelerates purine metabolism and raises uric acid. This is a dietary change that also aligns with broader cardiovascular health goals relevant to many furosemide patients.

Consume cherries or tart cherry juice. Emerging evidence suggests that anthocyanins found in cherries may reduce uric acid levels and decrease the frequency of gout attacks. While this should not replace medical treatment, it is a low-risk dietary addition worth discussing with your physician.

Alternative Diuretic Options for High-Risk Patients

For patients with a documented history of gout or significantly elevated baseline uric acid, it is worth discussing alternative diuretic options with a prescriber. Not all diuretics carry the same uric acid risk profile.

Torasemide, another loop diuretic, has been associated with a lower degree of uric acid elevation compared to furosemide in some studies, though it is not universally available at the same cost.

Losartan, an angiotensin receptor blocker (ARB) used in hypertension management, has a well-documented uricosuric effect—meaning it actively promotes uric acid excretion. In patients whose furosemide is prescribed primarily for blood pressure rather than fluid overload, a prescriber may consider whether an ARB could serve a dual purpose.

Spironolactone and other potassium-sparing diuretics have a more neutral effect on uric acid, though they are not appropriate substitutes for furosemide in all clinical contexts.

These conversations require individualized clinical judgment. The goal is not to abandon effective therapy but to find the combination that manages fluid retention while minimizing the metabolic side effects that erode quality of life—and financial stability.
