A 2020 analysis published in the Journal of the American College of Cardiology found that aggressive decongestion strategies—including higher furosemide doses—did not produce worse renal outcomes than conservative approaches, provided patients were monitored carefully and electrolyte imbalances were corrected.

The key phrase is appropriately managed. Furosemide misuse—whether through excessive dosing, inadequate hydration, or failure to correct potassium and magnesium losses—can create conditions that stress renal tissue. Hypokalemia, for instance, has been associated with tubular dysfunction when severe and prolonged. Dehydration-induced acute kidney injury is another real, if avoidable, complication. These outcomes are not inherent properties of furosemide; they are consequences of its misapplication.

The goal is not to counteract furosemide by drinking large amounts of water, which can actually blunt the medication's effectiveness and worsen fluid retention in heart failure patients. Instead, follow your physician's fluid intake recommendations, which may include a daily limit if you have heart failure.

Avoid nephrotoxic medications when possible. Non-steroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen and naproxen can significantly impair kidney function when combined with furosemide by reducing renal blood flow. Discuss pain management alternatives with your provider.

Adhere strictly to your prescribed dose. Self-adjusting furosemide based on perceived swelling or discomfort is one of the fastest routes to electrolyte imbalance and dehydration-related kidney stress.
