Most patients taking furosemide have heard the standard cautions: watch your potassium, avoid excessive NSAIDs, be careful with certain antibiotics. These warnings are well established and clinically important. But the American pharmaceutical landscape has changed considerably in recent years. Millions of patients are now taking medications that did not exist or were rarely prescribed a decade ago — and the interactions between these newer drugs and loop diuretics like furosemide are not always prominently featured in standard patient handouts.

What follows is a detailed examination of drug categories that warrant particular attention, organized not by severity alone but by how frequently they are overlooked in everyday pharmacy consultations.

GLP-1 Receptor Agonists: The Weight-Loss Medication Intersection

Drugs in the GLP-1 agonist class — including semaglutide (Ozempic, Wegovy), liraglutide (Victoza, Saxenda), and tirzepatide (Mounjaro, Zepbound) — have become among the most widely prescribed medications in the United States. They are used to manage type 2 diabetes and, increasingly, obesity. Patients on furosemide who are also prescribed a GLP-1 agonist may encounter a compounding dehydration risk that deserves direct attention.

GLP-1 medications frequently cause nausea, vomiting, and reduced appetite, particularly during dose titration. These gastrointestinal effects reduce fluid and electrolyte intake at exactly the time furosemide is promoting their excretion. The result can be a more rapid onset of dehydration, electrolyte imbalance, and, in some patients, acute kidney injury. Physicians and pharmacists do not always flag this combination because neither drug is traditionally listed as contraindicated with the other. The risk is additive and situational rather than pharmacokinetic — which makes it easier to overlook.

Patients using both should monitor for symptoms of dehydration (dry mouth, dizziness, decreased urination, unusual fatigue) and report them promptly, particularly during the first several weeks of GLP-1 therapy or following any dose increase.

Erectile Dysfunction Medications: A Pressure Problem

Phosphodiesterase-5 (PDE5) inhibitors — sildenafil (Viagra), tadalafil (Cialis), and vardenafil (Levitra) — are prescribed not only for erectile dysfunction but also for pulmonary arterial hypertension. These medications work by relaxing and dilating blood vessels. Furosemide, by reducing circulating fluid volume, also lowers blood pressure.

When taken together, the blood pressure-lowering effects of both drugs can combine to produce symptomatic hypotension: dizziness, lightheadedness, fainting, and, in severe cases, cardiovascular compromise. This risk is amplified in older men, in those who take furosemide at higher doses, and in patients who are already borderline hypotensive. The interaction is pharmacodynamic — meaning it is about overlapping effects rather than how the body processes either drug — and it can be unpredictable in timing.

This combination is not universally contraindicated, but it warrants an explicit conversation with a prescriber. Timing of doses, furosemide dosage review, and blood pressure monitoring are all reasonable management strategies. What is not reasonable is assuming the two are safe together simply because both are commonly prescribed.

Newer Anticoagulants: An Indirect but Real Concern

Direct oral anticoagulants (DOACs) such as apixaban (Eliquis), rivaroxaban (Xarelto), and dabigatran (Pradaxa) have largely replaced warfarin for many patients with atrial fibrillation or venous thromboembolism. These drugs are often perceived as easier to manage than warfarin because they do not require routine INR monitoring. However, this perception can lead to complacency about other interactions.

Furosemide-induced dehydration and hemoconcentration can theoretically increase the concentration of anticoagulants in the bloodstream, potentially elevating bleeding risk. Additionally, furosemide may displace some protein-bound drugs from albumin, which can transiently affect the free fraction of certain anticoagulants. While the clinical evidence on this specific interaction is not as robust as some others on this list, the combination warrants monitoring — particularly in patients who experience any unusual bruising, prolonged bleeding from minor cuts, or blood in urine or stool.

Perhaps more practically: dehydration from furosemide can worsen kidney function, and several DOACs are renally cleared. Reduced kidney function can increase DOAC levels and bleeding risk. This indirect pathway is clinically relevant and often underappreciated.

SGLT2 Inhibitors: Doubling Down on Diuresis

Sodium-glucose cotransporter-2 (SGLT2) inhibitors — empagliflozin (Jardiance), dapagliflozin (Farxiga), canagliflozin (Invokana) — are prescribed for type 2 diabetes and, increasingly, for heart failure and chronic kidney disease. They work in part by causing the kidneys to excrete excess glucose along with sodium and water — a mild diuretic effect in their own right.

Combining an SGLT2 inhibitor with furosemide creates a dual diuretic burden. While this combination is sometimes used intentionally in heart failure management under close supervision, in other contexts it can lead to excessive volume depletion, electrolyte disturbances, and acute kidney injury. The risk is particularly significant in patients who are elderly, have baseline kidney impairment, or are not drinking adequate fluids.

Patients who are started on an SGLT2 inhibitor while already taking furosemide should ask their prescriber explicitly whether a furosemide dose adjustment is warranted.

Lithium: A Classic Interaction That Remains Underreported

Lithium, used for bipolar disorder, has a narrow therapeutic window — the difference between an effective and a toxic dose is small. Furosemide reduces sodium reabsorption in the kidneys, and when sodium levels drop, the kidneys compensate by retaining more lithium. This can cause lithium levels to rise into the toxic range, producing symptoms ranging from tremor and confusion to seizures and cardiac arrhythmias.

This interaction is well documented in pharmacology literature but continues to cause patient harm, often because the prescribers managing each condition are different physicians who may not have complete visibility into the patient's full medication list. Patients on both drugs should have lithium levels checked more frequently, particularly after any change in furosemide dose.

Herbal Supplements: The Invisible Layer

Many patients do not think of supplements as medications and therefore do not report them to their pharmacist or physician. Several common supplements interact meaningfully with furosemide:

Building a Safer Medication Profile

The common thread across all of these interactions is that they are manageable when identified in advance and dangerous when missed. A comprehensive medication review — including prescriptions from every provider, over-the-counter drugs, and all supplements — is the single most effective preventive measure available to patients on furosemide.
